The requesting entity is listed as the Certificate Holder and the US Club Soccer Organization Member is listed in the description of operations.<\/p><\/li><li><p><strong>ADDITIONAL INSURED &#8211; FIELD\/FACILITY OWNER.<\/strong> This type of certificate is issued to a field\/facility owner used by a US Club Soccer Organization Member. The field\/facility owner is listed as the Certificate Holder and as an Additional Insured, and the US Club Soccer Organization Member is included in the description of operations.<\/p><\/li><li><p><strong>ADDITIONAL INSURED &#8211; NON-FIELD\/FACILITY OWNER.<\/strong> This type of certificate is issued to an Additional Insured who is not a field or facility owner. The requesting entity is listed as the Certificate Holder and as an Additional Insured, and the US Club Soccer Organization Member is included in the description of operations. Coverage does not extend to non-US Club Soccer programming.<\/p><\/li><li><p>This form must be submitted by an individual who has administrative rights within the applicable US Club Soccer Organization Member&#8217;s GotSport account.<\/p><\/li><\/ul><p><strong><span style=\"text-decoration: underline;\">2024-25 UPDATES<\/span>:<\/strong><\/p><ul><li>A <a href=\"https:\/\/gotsport.zendesk.com\/hc\/en-us\/articles\/23847239720727-Complete-US-Club-Soccer-Organization-Member-Certification-Form\" target=\"_blank\" rel=\"noopener\">2024-25 US Club Soccer Organization Member Certification Form<\/a> must be submitted and on file for your club\/organization before 2024-25 COIs will be issued.<\/li><\/ul><\/div><\/div><\/div><\/div><\/div><\/div><\/div><\/section>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section
