US Club Soccer provides accident medical-dental insurance coverage to its registered members participating in US Club Soccer-sanctioned and -authorized activities.     Please access https:\/\/portal.acitpa.com\/usclubsoccer\/<\/a>, create an account or sign into an existing account, then follow the instructions shown once you are logged in. Please be specific when describing injury and incident, our provider will need to know whether it is the right of left side appendage and the specific scenario of how the injury occurred. Providing all details will help avoid unnecessary claim processing delays.<\/p><\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

    1. Internal US Club Soccer verification.<\/strong> After you submit this form, US Club Soccer will review claim eligibility and verify the accident with the claimant\u2019s club\/member organization.

    2. Post-verification claim status update.<\/strong> You will receive an email with the claim status when the verification is complete. If approved, the claim will be processed by either A-G Administrators<\/span> (for accidents occurring through July 31, 2023) or ACI<\/span> (for accidents occurring on or after Aug 1, 2023).
